REPEAT MEDICATION REQUEST FORM
If your pet is on long-term medication for a chronic health condition and you would like to resupply you can use this form. Please note that in order to satisfy the law a veterinarian may request that you bring you pet in before any medications can be supplied regardless of the time since your pet’s last visit. Our team will call you to let you know if a visit is required.
We will review this form and be in contact either via phone or email, to advise you that your medication has been ordered, or to let you know it’s ready for collection, or to advise you that the vets have requested a scheduled visit or phone contact prior to dispensation.
